Creation of Single-Page PDF Form with action buttons

We have a requirements for a simple single-page PDF document to be converted into a form with the following attributes: - 'Clear Form' action button. - 'Save Document' action button - saves a copy of the document in read-only format with the document name taken from the concatenation of three data entry fields in the form. There also needs to be validation implemented for this button that prevents the save action from completing if one or more of the mandatory form fields is left blank. - Printing Formatting - when printing the document, the two action buttons should be hidden. The document should also be auto-formatted to print on standard US 'Letter' paper size.
