PHP Symfony2 OrangeHRM modification (2)

I have an existing OrangeHRM solution written in Symfony2, PhP, MySQL. There is a form which captures a users Sick Leave request. I have added a radio button to the form to select one of three additional values. I have added a new field in the DB. I want this selected value to be saved to the Leave Table (DB) when the Sick Leave is requested (submitted). On submitting the Sick Leave request, the new value needs to be written to the DB when the other values are written.
