I'm trying to build a code with specific outlines.. I have a copy of what the output should look like but I need help with the specifics

The application will utilize loop structures and if statement variations in addition to variables, constants, mathematical and relational operators, and Java API classes and their methods." Output spacing, formatting, and spelling are to match the example console output illustrated above, but of course the user input will be different.
