A very simple online form for a website.

We need an online form made which can allow a user to fill in details and upload a picture. Then upon hitting submit on the form an automatic email will be generated and sent to the relevant company. The email will contain; the answers submitted in the form and the uploaded picture. The email will need to be sent to the company. It will come from either our company email and CC in the user or it will be sent from the user and CC us into. When the user fills in the form they should get an automatic email from us to say thank you. When the email is sent it should contain: The filled out form, an attachment of the picture and some text stating: Please see the below form and attached picture. If you have any queries please contact the user directly. Allow user to answer 12 questions. These will be a mixture of drop down questions and free text. 1: name 2: address 3: company 4: … I will need a part to say they are happy to accept our Terms and conditions upon pressing submit. The form will be put onto our company website. The form doesn’t need to store any user data, other than being CCd into an email. We are predicting an average of 1000 people in the first 6 months will be using the form. Can you provide us with details on how to change questions if required? Can the form be used on a mobile ap both Android and IOS? If the user hits submit will the form go to the relevant company? If we change the website can the form be used for other sites? Is there a limit to how many people can fill in the form? Can you confirm we will be CC’d into any emails? Is it possible to set up delivery reports (not essential)
