Create an interactive table/calculator for my WP website

I need help creating a simple calculator feature on my website.
What I have:
- Wordpress website with a few pages
- Enfold theme
What I need (summary):
An interactive table (allowing the user to change some numbers) calculating a value, in line with the website's overall design.
What I need (in details);
(1) One slide on the page should be a slider -> a fixed photo with a semi-transparent overlay form (see e.g. [login to view URL] and press "free valuation" - you will see what I mean).
(2) Initially the semi-transparent form should include a simple question along the lines "What is the market rent of your property?", an input field, and a button.
(3) After pressing the button the form should change and it should show a table with calculations. The form should have some editable cells, with the result changing depending on the inputs, live. The form will be similar to this one: [login to view URL]
Obviously, the whole thing should be eye-pleasing and working also on mobile (responsive).
I will provide an excel spreadsheet with the exact calculations and the table.
You are welcome to use ready plugins (eg LayerSlider etc.). I am happy to purchase additional plugins if you think they will make the final result much better.
