Hello I need a small web application that is implemented with mysql DB, this application should provided by EJB,JPA,Entity method. Your IDE should be Netbeans also you can use Netbeans wizard to create base application ejb,jpa,entity. I will give you database schema and pages Mockups.(DB contain 5 table ) That application has 2 part member and guest. if you use wizard to create pages, you need to change those pages for memberID and some other value like current day. the main pages you should design are home,register,login,dashboard,search(simple one),relate members to other info(like picklist).
