Customise Facebook Pages design and functionality

Looking for someone with experience building and designing Facebook pages with rich interfaces and some built in functionality embedded within form controls customized on the Facebook pages to help promote more interaction between users and the page, eg. email forms, etc. Please provide past samples as this is the most important determining factor.
