Create a proper PDF from a Word.doc

Please note that I require a proper PDF to be created from the [login to view URL] attached. I do NOT want a simple conversion job done using a piece of simple software. I have Docudesk PDF Converter and could do this myself. Several bidders have already sent attachments for me to look at which are just "converted" and I will not be accepting any of these! I am looking for a PDF to be created using Adobe Professional design software. It is only a 1 page document (an application form). Very Important: I do NOT require it to be fillable, just to be designed attractively and based on the colours of the logo with it. Please attach a sample of similar work to be considered for this project so that I know you will design it properly. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
