Create a custom Filemaker database + mobile data entry form

This is for a model agency called [login to view URL] Dulcedo invites and meets with people (candidates) who wish to become models. We are looking to hire someone to develop a customized Filemaker pro 11 compatible database to help us with this process. To better understand what we need let me explain the steps. 1) Everything begins when a candidate visits the office and fills out a paper form with all sorts of information and questions asked. 2) a Dulcedo staff will take photos of the person using a digital camera 3) The paper form is then scanned 4) A folder on the computer is created and the scanned form, and digital images are dumped in it for review 5) Dulcedo staff can then see the candidate and decide whether or not to hire this person to become a model. We would like an interactive form candidates can fill out using an iPad that has filemaker go installer on it. Then using the same iPad a Dulcedo staff can take the appropriate photos of the person right into the same page. Then different Dulcedo staff can "vote" on the person to be rejected or invited. The database should keep rejects in an archive so that we may search thought it at a later stage. See attached image of a form design that would be suitable (still would need work, not final) Before going "forward" with hiring someone, I will make sure to make specific project specifications and design mock-ups of the look we want to have an accurate and final quotation.
