C# MVC 4 with MSSQL 2005 Membership Provider

I need a C# MVC 4 Internet Application that will get roles and membership information from a MSSQL 2005 database that has been initialized using aspnet_regsql.exe. This would be a dummy website, that allows for nothing else beyond logging in and out. I have been trying to get SimpleMembershipProvider to work and I can’t with my DB. So I think it needs to be ripped out altogether and another membership provider used in its place. But I’m not sure. I can’t get it to work, and I don’t feel like hunting through online forums anymore. Should be really straight forward setup. I’ve attached an empty DB, but you can also create your own with aspnet_regsql if you’d prefer.
