create a csv file

Hi, I require a python script to be created which is commented for future edits, It will load up and ask a user to input a value such as 'toilet brush' , once the value is entered an output csv file will be created adhering to a specific structure. Please contact me before bidding, as I will then send you the output file that I would like to be created, once viewing the output file you can then place a bid. To a python guru, it's probably a very simple task. Thank you
