We'll give you a complete schematic with pcb layout to cover your requirements along with a help note to assist you in assembling and calibrating it. It will be designed with easily available off the shelf components.
IMP: Please note that standard USB outputs can drop as low as 4.7V sometimes due to cable losses. If the 5V/150mA output should reflect the actual USB voltage then fine. However if you want a true 5V output despite losses and transients then the design will change a bit.
We will also give you a working demo board constructed on perfboard so you can evaluate it but this is purely complimentary so it should not be linked to the official deliverables. However if you want the stable 5V out irrespective of the input voltage then we will need to develop a professional grade PCB for the same and the pricing will change.
If you want the professional grade PCB as well, we can do it too for extra as per requirement

As per initial discussion, we will give you the same in a USB dongle sized form. There will be 4 output pins for the 5v, 3.3V, 3.6V and gnd. The 5V will have a 150mA limit and the remaining 3.6V and 3.3V will have a combined output of 450mA .
You can also specify individual current limits for each voltage pin if you wish.
Will offer you 2 designs, you can pick either or mix and match and go with whichever components are easier for you to source. Will try KiCAD design but will offer schematics in PDF with sample PCB layout.
