conversion access to php/mysql bis

Hi, we are looking to convert an access database to a PHP/MYsql oriented system. This little application written in access was designed to manage insurance customers for brokers. After convertion we will need to add some more fonctionnalities to the application. to download the original access database : [login to view URL] 15mb we also already have the mysql database done only need the PHP part that does the queries that are executed in order to retrieve or insert data from/in those 5 tables. 5 forms that are in the access database and 6 reports to gather statistical data. best regards Irwin
