Data Entry for Website Database

Hello, I am currently developing a website and i need someone to do some data entry for me. The website is written in PHP. This is how my website will works (this part is already done). Website's web form collects users information (name, D.O.B, address etc). This information will be saved to the database. This is how it will work. 1. User select the name of an institution from a drop down list. 2. All information saved on the database is automatically added to a template PDF. For example: [login to view URL] All the users information would be added to this PDF. What i need you to do. I have approximately 2000 different institutions, all with unique application forms. (they all have very similar fields e.g name, age, address. But will all be formatted differently). I need someone to take approximately 2000 PDF's and add them to my database and create a relationship between the PDF and the database. For example: First Name on the database should have a relationship with First Name on the PDF.
