I need a few things changed on [login to view URL]:
Set Russian as Default Language (it says it is, but it's not working).
Add a module for Retail/Wholesale Pricing
On the [login to view URL] page:
remove gender
Remove Company
Add IPC Agreement (text with an "Agree" radio button)
Change Date Format - Day/Month/Year
Add "Enroller Name or ID (Who referred you?)"
Add "Sponsor ID"
Change "Your Address" to "Shipping Address"
Remove Fax
Add a field for Autoship - text entry & Quantity
