Develop e-waiver & check-in system for kids' play center

I am opening an kids' indoor birthday party and play facility and would like to have an electronic waiver and check-in system in place, instead of having our customers fill out paper waivers every time they come in. We would have multiple kiosk PCs in place at the front of the store, where guests can electronically "sign" our standard liability waiver by checking a box and agreeing to the terms and conditions, enter their information such as name, address, phone, email, as well as childrens' names, ages, and dates of birth. All of this customer information should be automatically saved to databases for our marketing research, and emails will need to be exported and saved for easy (or ideally, automatic) upload to MailChimp, our email marketing service. The signed waivers should be saved individually and backed up automatically and redundantly. We should be able to search for any customer's waiver on the spot and be able to view it on a networked PC. There would be a couple of introductory questions as well to make sure the kids have socks on, etc. I would also like to be able to generate "frequent customer" cards with barcodes, so a parent could scan their keychain barcode, their contact information and kids' names would pop up so they could select them or add additional children, accept the waiver terms, and move on quickly without having to re-enter everything each time. Payment would not be taken at the kiosks. The kiosk would print a receipt with the customer's details, to be presented to the cashier for payment. Thanks and I look forward to hearing from you!
