ACT! Link to webpage with variables

I work for a recycling company that gets about 40 calls a day with customers wanting to schedule pickups. These are customers that we do business with on a regular basis so we have their information stored in our CRM software. When they call we pull up a custom in house webpage where we type in their name, address, what they have, etc and that schedules the pickup.

We currently use ACT! 2005 Premium for Workgroups for our CRM software and have a custom internal web page for scheduling.

What I am looking for is a button to put in an ACT! contact record. If I am looking at a customer record I want to be able to click a button that opens our custom web page and sends url or form variables like customer name, address, etc.

Basically its just a button with a web page link but I can't figure out how to do it.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
