Add column to db and create interface in admin section

I currently have a db that requires the adition and capturing of 1 more column of data. I also require an interface be built in my existing admin section that wil allow me to easily view,sort and filter the data, as well as export the data to excel. Time is of the essence and I need the job completed quickly. 1 or 2 days max.
To clrify further:
Clients enter their email address on either subdomain1 or subdomain2. Current db has columns and tables capturing their email, opt-in y/n, and an incrementing counter. I require a column identifying whether the user submitted via sub1 or sub2, and the date.
The interface should present the db data in a clear way that will allow me to search and filter the db data for all column variables and show results of the search / filtering.
Once the search/filter query is complete, I should be able to export results to excel.
