Create ''post feedback'' form + backend PHP script.

Hello, I need very simple (without any design) HTML form for sending user feedback to email. If it possible data should be posted without page reloading. Also I need server-side PHP script for processing this data. This script must be compatible with PHP 5.3 You can provide any partially-existed solutions if where is no any copyright restrictions.
