Hi, I'm looking for someone who is able to create a form with a connection to a sql database. I will send you a excel-sheet with all the rows and columns. Every time the form is filled in, they should be able to save it, send it by e-mail in plain-text and download it to CSV. User must be able to add extra rows in the form. The whole script must be protected by a username and password. Thanks
