Convert Access Database into PHP MYSQL very quick one

I have an Access 2003 database. attached are screenshots of the forms i need to convert from Access to Php and Mysql database format. there is a contacts table for every employee, with email, department, phone, bookmanagerID. And i have 3 other tables with few having bookmanagerID and email as the common between all the required tables. I need this done in the next 24-36hrs or soon enough.
