Certificate of Membership form with PHP GD image generation

I would like to have visitors to my site enter their name and the date and then have an image generated by php gd which would display a "certificate of membership" that could be downloaded or posted to facebook. the data would be entered into a sql database and emailed to me as well. the new member would also receive an email congratulating them and with the image attached. ## Deliverables People currently visit my site and enter their name, the date, and their email address in order to receive membership status. This happens via the phpformgenerator script. After submitting the information, a page pops up that shows them a certificate, but it's html composed of jpeg images and text with their name and the date. The name, date and email go into a sql database and are automatically emailed to me. I would like to change this so that after entering the data, a php gd script makes a custom jpeg of the certificate instead. I would still like the data to go into the database, but I'd like there to be a first and last name, and the date should be done with drop downs (month, day, year). so there would be five fields which would go into the certificate. additionally, there would be other fields which would only go into the database and the emails: their email address, their city and country of residence, and an additional field for comments (optional). both myself and the new member would receive an email with the posted information and the jpeg as an attachment. the jpeg would not be too large - maybe about 1000 pixels wide by 800 pixels high. or perhaps it could be a png. I would also like for there to be a feature for the new member to post the jpeg to facebook with an announcement that "[the member] just became a member of [my site]". i believe this is done with facebook connect? or perhaps with javascript.
