CGI Form work with AJAX integration

We have some forms that are done in CGI. We want them redone using AJAX to make them more neat and compact (using tabs)
## Deliverables
Our current interface looks like this [login to view URL] un/pw available via PM with coders who meet my requirments. There are two sections of the interface that are being redone, NOT the entire interface. In the interface click on each of the 3 items in the inventory. Each is very similar, but has some small differences. Then click on "Create multiple new items" from the menu and select all 3 providers, this large form that you see is also to be changed. Please be aware that there is 1 more provider missing from this demo version. So in fact it is 5 forms. One for each provider, 4 in total and 1 large form that has essentially 4 forms all next to each other. All of these need to be changed and put into Tabs using [login to view URL] There will be in total 7 tabs to break up all the data from the form as it is. All form elements are to be changed with CSS to look like this form [login to view URL] There will be one tab for uploading pictures, i want to use this [login to view URL]
