Create MySQL Database, Plug in Forms, and Create an Admin Area

I have 5 html pages that I want to get turned into php pages. These pages have already been sliced and have the css done on them but the forms are not set to input data into a database. I need someone to turn the html pages into php pages, make a database, and make an administrative password protected login area for the website owners to view the data. **Please View the "Detailed requirements" below as they contain much more detailed information.** ## Deliverables I have 5 html pages that I want to get turned into php pages. I want the header and footer to be php includes. I'm going to be referring traffic to the site using a query string that contains the referring search term (?kw=whatever_term) and want to use a session variable to pass along that search term to the database so that I can track what search term each conversion comes from. I want to be able to see this term when I look at the conversion list on the admin screen. Forms: *** Allow only letters, numbers, hypens, @ symbol, and periods to be input into forms for submission *** Need to figure out a way to keep bots and scripts from inputing data into forms. I don't want to do a captcha on the form submission but maybe set it up to where after 3 submissions from a location/user a captcha is required. [login to view URL] I have a website (images of pages can be seen here [login to view URL] the designer will be working with pages that are already html pages) that consists of Audit Page - First Name - Last name - Business Name - Email Address - Phone Number w/ext. - Type of Audit Needed Business Page - Business name - Contact's First Name - Contact's Last Name - Email Address - Phone Number w/ext. - Type of Business Personal Taxes Page - First name - Last name - Email Address - Phone Number - Years taxes Need to be filed for These forms need to be hooked up so that the data goes into a database and then that data needs to be displayed for the site owners on a password protected area of the site and should look something like this [login to view URL] Admin page should have clickable Filters so admin can sort users by Type of database entry (personal, business, or audit) like it is displayed in the image on [login to view URL] Alternating background colors should be used on the database results. Need to have site email up to 3 specified email addresses (administrator should be able to edit these email addresses) with notification of new entry when one is added to the db via an online form submission display up to 100 db entries per page .... start with most recent Future Updates: **(not included in this project but I will most likely use the same programmer to do these upgrades when the time comes)** ***Add notes box - store old notes for display along with user that entered them and add a time stamp to the entry ***Multiple User Login ***Different Privilege Rights for type of user (admin/employee) ***DB Backup w/download option (only available to Admin) ***Disable Print and Print Screen Options for users that are not admin) ***Display Time stamp on new entries into db ***Search Box where you can specify what field(s) you want to search or search entire db for the information ***Manual Entry - Admin can input and/or edit information in fields ***Export Data Option (Admin only) so that it can be uploaded into a email marketing application/site like Constant Contact or MailChimp
