Application Change

Customer application that allows users to purchase Key Tags. The system works but we need to add a page so the customer can add their own information after they receive the tags. The application already has an admin area which has an add a new customer form but you must be logged in to add a customer. We want it so you do not have to be logged in, the form will look up to see if the tag number entered is already used. This is a function already in the admin programming. The database requires the addition of two feilds added a hidden date field and a promo code text field added to the customer record. The database can be access through cPanel MyPHPAdmin. Completed by Friday. We need a page that we can link to from the home page. The application is in the app directory. No graphics required
