Build PHP/MySQL Database around simple HTML Templates

Please see attached screenshot. I have several HTML forms for Construction Project Management, including: - Daily Health and Safety Checklist - Monthly Health and Safety Checklist - Daily Employee Timesheet etc etc I have already created the HTML templates for them (as per screenshot, for example), so you do not need to do that. I will provide them for you. I require a skilled website designer to create a PHP backend for the templates, to save the data into a MySQL database where each form has a table and has logical connections between tables. You will need to create both the PHP backend and the MySQL database. You will require some initiative and logic to create a relational database where you make tables such as Supervisors (as each form links to a supervisor), Employee (as many Daily Time Sheets can have one Locations (as each form related to a specific location) etc. The PHP backend needs to have validation, error checking and must be clean, reliable PHP code. The end result: The forms will save to a logical, relational database A neat HTML page will list (from the database) all the entries under each category of form. So you can retrieve Daily Timesheets, for example, for any date, or for any employee or supervisor, and view them all in a list and individually.
