Add multi file fields and progress bar to upload form

I have an upload form that includes a menu to select which folder to upload to and is accessed by user login. Currently there is only one file field and I need 3. Also required is a progress bar when the form is submitted. My application has a choice of 2 upload forms that the user can choose from. I only need the work performed on one of them. Form #1 already has a progress bar when uploads are made. But there is only one file field. Form #2 is a more basic upload form for javascript disabled users. This is the form being currently used. However, if it's easier to perform the work on form #1, I am fine with that as long as it looks like the basic form when you're done. I think it would make more sense to implement the changes on form #1 since the progress bar is already there. But of course, you decide. The admin has an identical upload form as the user. So whatever is created will need to be copied over to his form as well.
