Design PHP/MySQL Website(repost)

I need a website that has about 7 screens (the number of files would depend on personal programming style). I need someone expert in PHP/MySQL & CSS and some Javascript. The CSS needs to be extrapolated/removed from the HTML completely (no CSS in the HTML - the CSS must be a completely separate attached style sheet). No graphics are necessary (those are done already). The database is also already designed, although there may be some additional fields to add that I haven't thought of yet. The CSS style sheet is done, but may require slight modification. The home page is done already. The following screens remain: 1) login screen 2) submit form screen 3) view stats screen 4) search database screen 5) view search results screen 6) access dbase record screen The site basically needs to allow a visitor to create a login email/password combo upon first visit, track returning visitors via cookies, send emails to multiple email addresses upon a form submission (the contents of the form formatted as an email message and sent to multiple emails in the dbase based on certain match criteria). Javascript checking if form is complete required. Some simple stats will need to be updated each time data is accessed by a user. Dbase records need to be displayed in table form on some screens. If a dbase record is accessed, an email needs to be sent to the person who submitted the form. I would expect this job to be completed in just a couple days. There is nothing fancy/hard to do for someone that knows PHP/MySQL and CSS. I could do it myself but don't have the time. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform All HTML/CSS must appear correct in all browsers on both Mac & PC. Server site resides on is Linux with cPanel admin. PHP & MySQL for Linux installed on server currently.
