Add "add sample product to cart" functionality in Magento

For our Magento store we need to add a functionality to our product page that allows customers to order a sample of the product by adding it to the cart. Please read the below description carefully. If you have questions do not hesitate to ask. If this project is completed successfully the developer can expect more custom work to be done in the future. Please include references of similar work that has been done. We are looking for someone who can: 1) Add a link to the product page to add sample product to cart. 2) When clicking on the link a popup should appear. The popup should look similar to the popup appearing when clicking on the "add sample to cart" button on this page: [login to view URL] Personal information gathered from customers (such as the last two fields in the example link) should be visible in the back-end under "customer data". 3) Create a pricing rule that will allow customers to add up to 3 free samples to their cart. Additional samples can be added at a price. NOTE: - The price of the sample will be different from the price of the regular product. - Related products functionality is already used in the site and should remain working. - Popup should work in all browsers, including tablets. - All text should be translatable in the translation file.
