Create a Mobile Website

the first part I scooted a screen shot in that would be what I would like to see.

Cardlogix made the app in the video and they will send the data to use via a web service where we write it into our database using C# app or MIRTH.

I need to have a Splash screen with Login (ASP.NET Membership), second screen like there first screen but with Search and Add Patient. I need the new app to read the data back and display or edit. I need to have a search option using NationalID or LastName or DOB. I need to be able to add patients. For patients found during a search I would like to be able to update the data or photos using cam on phone / laptop.. They MIGHT NOT make the final app and I will be stuck adding the New Patient and Update features so I would like to be ready. I would like it in AZURE if possible in C# or VB .NET with a new outlook account that could be usedas a mobile website. We can use 1 table for now assuming only 1 kid for now and I have all the fields they sent us in 1 table. The create SQL table with the fields I understand to be used below, they should match the fields in the video. I have not checked all these fields against the demo app to see that they followed the field list.
