I have a website - [login to view URL] (Wordpress). I need to add a form widget towards the top of this page, probably to the right next to my you tube video, where a person can enter their name, company name, and email address and then download a one page word document. I'd like the widget show/say (Free Download - Safety Policy Sample) and have a pic of the doc or something enticing like that). And example of a widget layout I like is: [login to view URL] - The widget I like is on the right side, titled "Want a Free Experience Mod Review?"
