You will need to re-create that page and functionality from scratch. It will need to be fully responsive. The logo and the green circle images will be provided (I'll also provide the code for the MailChimp form). Other images - man in suit, triangle for the form and thumbs-up sign you'll have to provide yourself or use a stock photo.
Page load time is important and will be checked using Google's Page Speed Insight tool. This is a simple design without flash/large images and so I'd expect it to score high on the speed test (in the "green" zone for both desktop and mobile).
Extensive further design work if you provide an on-time and high quality service.
