Build 1 Responsive Data Capture Page

This is a 1 page responsive page where the data gets saved to a MySQL database.

I'm sure you understand by now that this page should be fully responsive.
So, the process is simple:
A email will be sent out to users. If they press accept, then they get to the 'accepted' page and they sign up. Their details gets saved in a MySQL database. If they press decline, on the email, then they land on the 'decline' page, where they need to explain their reason for declining. you need to capture this information against their email address. The URL in the email will carry their email address and their choice in a variables to your page. You need to autocomplete the email field. When they submit their data they see the thank you overlay.
That's it.
All your work needs to be tested in desktop browsers and mobile devices.
The drop down styling should be done as per the designs above. I need this completed by Wednesday 20 April, end of day, so that we can start testing.

1. We will only release milestones if the work is done properly and tested properly.
2. We want to test the HTML and SQL on our own server before releasing any milestones.
