Build a Comment Form

I need you to create a simple HTML5 form which allows users to input a comment, and comment title. Users can also respond to a preexisting comment similar to a comment thread. I need the data to be posted on the form immediately (PAGE SHOULD NOT RELOAD, and I need to see the post immediately) and you should incorporate AJAX and JQuery for this purpose. The "reply" link (see attached image) should automatically open up a text-area with its own submit button. Bonus ($10): If you create a basic registration/login form to capture different users. That way, users don't have to enter their email/name on the comment form. This will help me immensely! User Photo: Users don't need to upload photo, however, for sake of testing, users should be able to select an icon/avatar which is associated with their username in the comment area. You can add this wherever you want. Backend (mySQL) should capture: BlogID (every blog as its own comment area) CommentID ThreadID (this corresponds to subcomment) UserID UserIcon Title Description CreateDate IsActive Also, capability to delete a comment. If user deletes a comment, it should still capture the comment, except make it in active in the backend. Code should be simple, clean, and well commented. This is a VERY simple project which experienced developers can probably do in their sleep. :) Acceptance Criteria: Upload this to your test server.
