Build Custom Excel Form

Freelancer must have Skype and have communication throughout this short project. Mileage Form: The Excel Mileage Form should be duplicated (exact same formulas for mileage calculation, same colors, font, etc.). We are only reproducing this form because it is locked and we need another tab named "CAP". CAP Requirements: - The CAP report should be similar to the Word document, but in Excel. Instead of checkboxes for service rendered, the new form should offer a drop down. Program (service rendered) dropdown choices are: 56A (Transportation), 56E (School Transport), 71A (In Home Case Management-Licensed), 71Q (In Home Case Management-Masters), 71B (In Home Case Management), 95A (Intensive Therapeutic Services), 00W (Hoteling) - Multiple Programs/Services should be able to be selected - Time (Start and End) should be a field located beside the Program Name (excel should auto calculate time) - Sometimes there will be a need to enter 2 start and end times for 1 program - A field for mileage should also exist (no calculation required, we should just be able to enter mileage) - Type of contact options should remain a checkbox. Options are: Face to Face, Telephone Contact, Missed Appointment - Form fields should be created for all entries - The Summary section should allow as much content as the writer desires. The summary section should allow the writer to fill up the first page and automatically create a field on the second page. - Page size is 8.5x11 (US standard) - Change "Signature" to "ACS Worker" and allow a form field for worker to type their name and title below. - Updated address is: PO Box 70123 Albany, GA 31708 (888) 576-8372 (866) 794-8372
