Access Database Program and Reports Needed

I require a simple access database program where I can capture the following information
Screen 1: I would like to input Customer information:
1. Name
2. Company
3. Address
a. City
b. State
c. Zip
4. Phone
5. Email
6. Processing Date
Screen 2: Capture the following information.
I will usually scan 200 Hard drives at a time. The information I want to capture is:
1. Barcode of the hard drive (Field Must be able to scan into)
2. Scan Part Number (Field must be able to scan into)
3. Manufacture of the Hard Drive ( I want this as a drop down field that I can prefill Hard drive Manufactures in. Common manufactures that will be part of the drop down will be:
a. IBM
b. Fujitsu
c. Western Digital
d. HP
e. Hitachi
f. Maxtor
g. PMC
h. Quantum
i. Samsung
j. Seagate
k. Unknown
I require reporting functionality to easily supply my customers with. I need my company logo and contact information to be included in this report along with the customer information. The report title should be Secure Hard Drive Audit Report. The information that we collected will follow. Please bid. I need this job completed for Sunday Jan 25th 2009 Est time 12pm.
