Affiliate link/banners/form/database

Setting up an Affiliate structure to? bring in traffic for my business.

I need a Banner or a Link created to allow my Affiliates to place on their site to bring customers to my site.? I also need the entry form created to capture the customers return contact info and the Affiliate Link info so? I know which Affiliate referred the customer.

I will also need a database to store this info.

I also need instruction on how to change the link info for each new Affiliate.

I currently use 1and1 hosting

The entry form should be on the landing page.? I? would prefer when a customer clicks the link to come to my site, that it will not completely direct them away from my Affiliates site, but rather just? open a new box.

I am a novice at this so please let me know if you have better ideas.
