The template should meet the following: - Responsive, mobile-friendly (bootstrap preferred) - Single page website with frozen top navigation. An example of such a page can be found here: [login to view URL] - A maximum of 4 colors used for both text and background. No strong colors (light blue and white should set the theme, other colors in the indicated template can be used - except green) - An sample input form comprising all common field types (text, text area, e-mail, date, etc.) with working validations. - Form to use simple JS (plain jQuery) - The first section of the page should allow for a full-width banner image - Scroll effects as in the indicated template. - A placeholder for a page icon - All styles should be included in one simple css file.
