Design packaging for a seminar DVD about anxious children

Contest Brief

I am seeking a design to use for a DVD of a seminar that was videorecorded about 'selective mutism' (ie., kids who are so shy they don't talk at school. It is similar to social anxiety).
The designs needed are:
1) Front and back cover for a DVD case
2) Circular picture to be printed on the DVD itself.
3) Menu to show when the DVD starts to play
I have included attachments of the layout and dimensions required for the jacket and DVD.
I have also included the designs of our previous two DVDs, which were done in-house and not by a graphic designer. The current design should be not too dissimilar to those, but seeking a more professional look.
The current designs required are for a DVD of a seminar that was given this month titled 'What all staff need to know about selective mutism: A staff inservice'. The seminar was about children from preschool to high school age. I would prefer a photo of a child between 9 to 12 years of age (i.e., late primary school age). The seminar was about children who are so shy they cannot talk at school, and is training for all school staff so that they understand the condition, what causes it, and how they should respond to a child like this at school.
Format required is pdf, as well as a format that can be edited and converted to pdf (Adobe InDesign or word)
I have attached a word document with the menu text required when the DVD starts to play.
I've included a photo of the presenter, Elizabeth Woodcock (that's me)

On the back cover, the blurb about Elizabeth Woodcock can stay the same as the other two DVDs.

After the blurb, the text should be:
Selective Mutism is a complex but treatable anxiety disorder which affects children's ability to communicate at school and in other social situations. It is important for all school staff to understand about selective mutism so that they know how to talk to and respond appropriately to these children.

This 50-minute seminar provides strategies and information to help all school staff in a preschool, primary or high school setting:
* Understand selective mutism
* Understand the role of anxiety in selective mutism
* Understand factors in the school environment that make it harder or easier to communicate
* Learn how to respond to and communicate with children with selective mutism

Any photos need to be legal for production and sale of the product.
All fonts must be embedded in the pdf format
Ensure there is a 3mm bleed around every edge.
All artwork must be print ready and set up as CMYK
The final artwork must be a flattened PDF file, with the template on a separate layer.

      Hi. Yes I'm not that familiar with all of this, but I think that's okay. I had attached a document from the people that will publish the files with their requirements, so it should state it there. Thanks.

    Sorry, opening the text-document i can not see the menue-text, the document only contains the green rectangle with the name of the clinic. Please, place the menue-text into the description like the text for the back cover.

    Is that the logo of the clinic should be used? Because different fonts are used in the examples.
    Is it possible to add a logo file like PDF or EPS?
    Do you prefer stock photography or can it also be visualized?

      Sorry which text document do you mean? I've attached a range of documents such as pdfs and a word document but no text document. You might mean the word document - I'll put the text for that now into the description. I am fine with stock photography or whatever. Yes please use the logo of the clinic, I'll upload something. Thanks for your questions.
